When you come to our office for an interview or to fill out an application, please be sure to bring with you:
Social Security Card
Proof of Citizenship
(Passport, Permanent Resident Card, Certificate of Naturalization,
or Photo ID and Birth Certificate)
Home Health Aide license from the State of New Jersey
(if applying for certified home health aide position. Not necessary for companion
positions.)
The names and phone numbers of two professional references. (Former employers)
Certified Home Health Aides also need a physical, MMR, and TB test. If the TB test is
positive you will need a chest x-ray report. |
